Performances must be appropriate for all ages. Time limits and other info are included in the application.

If you would like to perform at the Utah Summer Dance Festival (USDF), please complete this application form and send payment to utsummerfestival@gmail.com. Once we review your application, we will follow up with an email confirming your registration. Please note that ALL performances must be family-friendly and appropriate for all audiences.

Solo/Duet - 6 minutes or less ($5.00)
Trio/Group 8 minutes or less ($10.00)
Teacher Section - up to 15 minutes ($20)
Large Multicultural Group or School (contact USDF)
Musician or Band (contact USDF)

Register each performance set individually (i.e., if you want your group to perform and you also want to perform a solo, complete two applications.)
The Teacher Section is for back-to-back performances up to 15 minutes (generally used by schools who don't have costume changes).

Check with dancers to find out what other groups they are performing with. We will do our best to separate groups to accommodate costume changes.

By submitting this application, I agree to release, indemnify and hold harmless the Utah Summer Dance Festival Committee, Desert Journey School of Dance, South Salt Lake Community Center, and all agents, employees, owners, contractors, volunteers, students, and landlords from any and all claims of liability, actions, demands, or judgments from any losses or damages, or in the event of an injury or any medical problem or condition that may arise from participating in any Utah Summer Dance Festival activities. I understand that the activities I am applying to take part in are completely voluntary and assume all risks and obligations. I affirm that all performers in my group or school agree to this release and indemnity. *
